Safety Culture

Safety culture has been an integral part of the way organisations have achieved safety for the last 30 years.  This paper introduces the concept behind safety culture and then uses the examples of the Challenger and Columbia disasters and the unintended acceleration problems of Toyota to show how large organizations can lose their way in this area.  It then provides guidance to help the reader identify if the organization where they are working needs to take a look at their safety culture.
